Take a patient's information once. Every form after that fills itself.

altaFlow builds the intake packet, the consent form, and the credentialing file from information your forms already collected, routes each one for signature, and writes the result back to the record. No code, live in weeks.

What patient access, clinical operations, and credentialing stop doing by hand

Staff stop typing the same information twice.

Take a patient's information down once and trust it is right everywhere we keep it.

The intake form writes straight into the record system, so the signed consent and the chart never disagree about what the patient answered.

Workflows

  • Patient intake and registration
  • Consent and authorization capture
  • Release of information requests

Work that waited days finishes in minutes.

Get everything signed by everyone who needs to sign, without chasing people all week.

Packets route to the next signer on their phone, remind the ones who stall, and let any staff member answer where a file is sitting without opening four systems.

Workflows

  • Provider credentialing and recredentialing
  • Business associate agreements
  • Policy attestations

Census spikes stop requiring temporary staff.

Get every new patient and every new provider set up and ready to go, even in the busiest weeks.

The whole intake journey runs itself, from the first form through document uploads, conditional packets, signatures, and filing.

Workflows

  • New patient onboarding
  • Provider and vendor onboarding
  • Prior authorization document collection

Where the intake actually breaks

A patient fills out a form in the waiting room, and a staff member types the same answers into the record system an hour later.
A credentialing file sits for a week because nobody can say which signature it is waiting on.
An audit request arrives and someone spends days assembling consent evidence out of email and scanned folders.

From the first intake form to the filed record

Step 1

Capture

altaflow-healthcare-capture-patient-intake-form

A patient, guardian, or provider completes a branded form on any device, and conditional fields hide every question that does not apply to them.

Step 2

Generate

altaflow-healthcare-generate-consent-and-financial-responsibility

altaFlow assembles the packet from what they submitted, pulling the right consent language, attachments, and forms for that visit type or specialty.

Step 3

Route and sign

altaflow-healthcare-route-consent-and-financial-responsibility

Each signer receives the document in order, with reminders, and internal approval gates run before anything leaves the organization.

Step 4

Write back

altaflow-healthcare-write-back-box

The completed document and its data land in the record system, filed where your health information team already looks for them.

Build

Webforms

Branded intake and consent forms with field-level conditions, so a patient only answers what applies to their visit.

Document Generation

Consent forms, credentialing packets, and authorizations built from submitted information, with specialty-specific sections added automatically.

E-sign & Fill out

Signing on any device, in close to 30 languages, for patients and families who do not share a first language with your staff.

Automate

Workflow Configuration

Conditional steps and dynamic document sets, so pediatric or behavioral health paperwork appears only when the answers call for it.

Automation Bots

Pre-fill bots move information between forms, and scanned-data extraction pulls details off an uploaded insurance card or identification document.

Notifications and Reminders

Scheduled reminders and text-message delivery reach the patients and providers who never open email.

Integrate

Native CRM Integrations

Two-way sync with your customer relationship management system, Salesforce, Microsoft Dynamics 365, or NetSuite, plus Microsoft 365, SharePoint, and Google Workspace.

Integration Bots

Move information and finished documents into the record system and the platforms your billing and human resources teams run.

APIs for Developers

A documented application programming interface, so intake, generation, and signature can be embedded into a patient portal or a scheduling app your own team built.

Manage

Document Tracking and Audit

Real-time status on every packet and a complete activity log behind it.

Team and Access Management

Roles and workspaces per department, so patient access, credentialing, and compliance each run their own workflows.

Security and Authentication

Single sign-on through your identity provider, plus two-factor authentication on every account.

It writes back to the systems you already run

Whichever system the process lives in, the finished document and its data go back there.

Start Fast with Prebuilt Healthcare Workflow Templates

Security at the core

Enterprise-grade security and compliance

SSO, role‑based access, audit trails, data retention, and encryption. Built to support regulated use cases with governance across every workflow.

“I like the best the fact that the software is so simple to use yet so powerful. With a few clicks of a button, you can create multipage/document forms that are pre-populated based on information users already supply. The fact that somebody only needs to enter their name once and it replicates throughout the whole system is just amazing.“

Frequently Asked Questions

Which record systems do you integrate with? altaFlow connects natively to Salesforce, Microsoft Dynamics 365, and NetSuite, and through integration bots to Microsoft 365, SharePoint, and Google Workspace. For an electronic health record or practice management system, the connection is built against the interface your organization already exposes, and the completed document plus its data are written back to the patient or provider record. Bring your system to the demo and we will show the write-back running.
How do you support our privacy obligations? Every document carries an audit trail with the signer, the internet protocol address, and the timestamp. Access is controlled by role, team, and workspace, so only the staff who need a patient's file can open it, and retention rules dispose of records on the schedule your policy sets. Handling is HIPAA-supported, and a business associate agreement is part of the contracting process.
Can a department build and change its own workflows? Yes. Patient access, credentialing, compliance, and human resources each get their own workspace, and the person who owns the process builds the form and the routing in a drag-and-drop builder. Adding a signer, changing a question, or updating consent language takes minutes and does not require a developer or a support ticket.
How long until we are live? Weeks, not quarters. Setup time drops from more than 100 hours to 20 to 30, roughly 80% faster than building the same thing by hand. Results vary. The sequence that works is one process first, then the rest of that department, then the next department.
Do you handle consent in more than one language? Yes. Forms and generated documents can be fully localized, or logic can show the right fields based on the language a patient selects. The signing session itself is available in close to 30 languages.
How are signed documents archived? Signed files attach to the record in your system of record, or archive to SharePoint, Box, or another repository, following the retention policy you control. Every version is tracked, and only the current form can be signed.
How is it priced? Do we pay per signature? No. Pricing is a site license with unlimited users, so a census spike or an open enrollment period does not raise the bill and nobody has to ration signatures. Per-envelope pricing does the opposite: the meter tells you to send fewer documents in exactly the weeks you need to send more.
We already have an eSignature tool. What does altaFlow replace? Signature is the last step, not the process. altaFlow covers five capabilities in one platform: document generation, PDF editing, workflow automation, contract management, and eSignature. Organizations consolidate five separate tools into one at 30 to 40% less than the incumbent stack. If you want to keep the signature tool you already run, ask about the integration path in the demo, because it depends on the tool.

Bring the packet that takes your team the longest.

In thirty minutes we build it, run it, and show you the data landing back in your record system.
